RELEASE AND WAIVER OF LIABILITY
In consideration of receiving tattoo or piercing services from Body Gallery Tattoo and Piercing LLC, I, the undersigned, hereby acknowledge and agree to the following terms:
ASSUMPTION OF RISKS: I understand that receiving a tattoo or piercing involves inherent risks, including but not limited to pain, discomfort, allergic reactions, infection, scarring, and dissatisfaction with the final result. I acknowledge that I have been informed of these risks and accept them willingly. I have been educated on proper aftercare procedures for my new tattoo or piercing and agree to follow the instructions provided. I understand that Body Gallery Tattoo and Piercing LLC is not liable for any complications resulting from improper aftercare on my part. I also acknowledge that the studio follows strict sterilization and hygiene protocols to minimize the risk of infection. I understand that proper aftercare is essential for safe and healthy healing.
HEALTH CONSIDERATIONS: I affirm that I am in good health and have disclosed any relevant medical conditions, allergies, or medications that may affect the tattoo or piercing process. I understand it is my responsibility to notify the artist of any changes in my health status before or during the procedure.
AGE VERIFICATION: I confirm that I am at least 18 years of age and possess the legal capacity to consent to tattoo or piercing services. If I am under 18, I certify that I have obtained the required consent from a parent or legal guardian, who is present with me at the time of the procedure.
DESIGN APPROVAL: I have reviewed and approved the design and placement of my tattoo or piercing. I understand that any changes requested after the procedure has begun may incur additional charges.
LIMITATION OF LIABILITY: I hereby release and hold harmless Body Gallery Tattoo and Piercing LLC, its employees, artists, and affiliates from any and all claims, demands, liabilities, or actions arising out of or related to the tattoo or piercing services provided. This includes, but is not limited to, any injury, damage, loss to person or property, or death, whether caused by negligence or otherwise.
